• Please include Polite Titles for each name, even those that seem apparent

• The 2008 Local Church Official Form is exactly the same as last year's except for the "2008" in the title.  If you already have it in PDF, Word, or RTF, you can just make sure the dates are updated and move the classes if you prefer, rather than re-typing the whole form... just make sure the dates are updated correctly!

• The rest of the forms are essentially the same as those used for the last two years, so on those where much of the information is the same you can re-use those that are saved already on your computer.

Saving Data in Completed PDF Forms

To save the data you enter in a PDF form requires use of a different program than the Adobe Reader.   Click here for more info about freeware, shareware, and commercial PDF form filling programs that enable saving of data.  The free Adobe Reader won't let you save the data you enter, but some other programs will.
