Maintain liability insurance on house and vehicles

Have reliable transportation

Have a working telephone

Have no more than 5 children already in your home

be a minimum age of 21

Be in good physical and mental health

Pass a criminal background check

Home must pass a health and safety inspection

Provide necessary personal documentation

Provide references from 2 relatives and 3 acquaintances

Understand and agree that corporal punishment IS NOT to
be used at any time

Complete 24 hours therapeutic foster car training
annually

Meet weekly with social worker and therapist

Keep daily behavioral and medication logs

Attend monthly training sessions provided by the agency.
